مقایسه اثربخشی tDCS و tPCS همراه با تمرینات زنجیره حرکتی بسته بر کنترل تعادل عصبی-عضلانی در استئوآرتریت زانو: یک کارآزمایی بالینی تصادفی

چکیده
اختلال در کنترل پاسچر و افزایش نوسانات مرکز فشار از پیامدهای شایع استئوآرتریت زانو است که می‌تواند خطر سقوط را افزایش دهد. بر این اساس، مطالعه حاضر با هدف بررسی اثر تمرینات زنجیره حرکتی بسته همراه با تحریک غیرتهاجمی مغزی بر کنترل پاسچر زنان مبتلا به استئوآرتریت زانو انجام شد. در این کارآزمایی بالینی تصادفی‌سازی‌شده و دوسوکور، ۵۴ زن مبتلا به استئوآرتریت زانو در سه گروه تحریک جریان مستقیم فراجمجمه‌ای (tDCS)، تحریک جریان پالسی فراجمجمه‌ای (tPCS) و تحریک ساختگی (Sham) قرار گرفتند. مداخله شامل ۷ جلسه تحریک مغزی و ۱۲ جلسه تمرینات زنجیره حرکتی بسته طی چهار هفته بود. شاخص‌های مرکز فشار در شرایط چشم‌باز و چشم‌بسته، پیش و پس از مداخله ارزیابی شدند. تحلیل داده‌ها با استفاده از تحلیل واریانس با طرح مختلط نشان داد که اثر اصلی زمان برای شاخص‌های مرکز فشار در هر دو شرایط بینایی معنی‌دار بود (P<0.05)، که بیانگر بهبود کنترل پاسچر پس از مداخله در تمامی گروه‌ها است. با این حال، تعامل گروه × زمان برای بیشتر متغیرها معنی‌دار نبود (P>0.05). تنها برای متغیر طول مسیر جابه‌جایی مرکز فشار در شرایط چشم‌باز، تعامل معنی‌دار گروه × زمان مشاهده شد (P<0.05). با توجه به دریافت تمرینات زنجیره حرکتی بسته توسط تمامی گروه‌ها، سهم مستقل تمرین و تحریک غیرتهاجمی مغزی در تغییرات مشاهده‌شده قابل تفکیک نیست. بنابراین، یافته‌های حاضر نشان می‌دهد که پروتکل‌های مبتنی بر تمرینات زنجیره حرکتی بسته می‌توانند در بهبود کنترل پاسچر مؤثر باشند، در حالی‌که اثر افزوده تحریک غیرتهاجمی مغزی در شرایط مطالعه حاضر محدود بود و نیازمند بررسی‌های بیشتر است.

Impaired postural control and increased center of pressure (COP) sway are common consequences of knee osteoarthritis and may contribute to a higher risk of falls. Therefore, this study aimed to investigate the effects of closed kinetic chain (CKC) exercises combined with non-invasive brain stimulation (NIBS) on postural control in women with knee osteoarthritis. In this randomized, double-blind clinical trial, 54 women with knee osteoarthritis were assigned to three groups: transcranial direct current stimulation (tDCS), transcranial pulsed current stimulation (tPCS), and sham stimulation. The intervention consisted of seven brain stimulation sessions and twelve CKC exercise sessions over four weeks. COP parameters were assessed under eyes-open and eyes-closed conditions before and after the intervention. Mixed-design analysis of variance revealed a significant main effect of time for COP variables under both visual conditions (P<0.05), indicating improvements in postural control across all groups. However, the group × time interaction was not significant for most variables (P>0.05). A significant group × time interaction was observed only for COP path length under the eyes-open condition (P<0.05). Since all groups received CKC exercises, the independent contributions of exercise and NIBS to the observed changes cannot be clearly distinguished. Overall, the findings suggest that CKC-based interventions may contribute to improved postural control, whereas the additional effects of NIBS were limited under the present experimental conditions and require further investigation.
