مسیر مرکز فشار راه رفتن و دویدن در بیماران با سابقه بازسازی لیگامنت متقاطع قدامی

چکیده
پس از عمل بازسازی لیگامنت متقاطع قدامی، همچنان تغییراتی در مکانیک دویدن باقی می ماند. بررسی مسیر مرکز فشار یکی از متغیرهای نشان دهنده مکانیک کلی راه رفتن و دویدن است. هدف از مطالعه حاضر مقایسه مسیر مرکز فشار در افراد با سابقه جراحی بازسازی لیگامنت متقاطع قدامی بود. 48 بسکتبالیست مرد (24 نفر با سابقه جراحی و 24 نفر سالم) در این مطالعه شرکت کردند. مسیر مرکز فشار هر دو پای آزمودنی ها طی راه رفتن و دویدن با پای برهنه توسط دستگاه فوت اسکن در دو محور داخلی-خارجی و قدامی-خلفی ثبت و در 4 زیرفاز مرحله سکون محاسبه شد. نتایج نشان داد که حین راه رفتن و دویدن در گروه با سابقه جراحی در فاز پوش آف جلوی پا مسیر مرکز فشار داخلی تر از گروه سالم بود. همچنین در گروه با سابقه جراحی، مسیر مرکز فشار پای آسیب دیده در فاز پوش آف جلوی پا حین راه رفتن و دویدن داخلی‌تر از پای سالم بود. از یافته های پژوهش حاضر می توان نتیجه گیری کرد که مسیر مرکز فشار افراد با سابقه بازسازی لیگامنت متقاطع قدامی با افراد سالم متفاوت است. این تفاوت در مسیر مرکز فشار می‌تواند باعث تغییر بارگذاری زانو شده و خطر ابتلا به استئوآرتریت زانو را افزایش دهد.

Gait mechanics alternation remain after anterior cruciate ligament reconstruction (ACLR). Evaluating the center of pressure (COP) trajectory is one of the variables that reflects whole mechanics of walking and running. The aim of this study was to compare the COP trajectory in people with a history of ACLR surgery. 48 male basketball players (24 with history of surgery and 24 healthy) participated in this study. The barefoot walking and running COP of both legs of the subjects was recorded by the footscan device in two medio-lateral and antero-posterior directions and was calculated in 4 subphases of the stance phase. The results showed that COP trajectory in walking and running was more medially directed in the forefoot push-off subphase in the ACLR group. Also, in the ACLR group, COP of the injured leg in the forefoot push-off subphase during walking and running was more medially directed than the healthy leg. Regarding the results of the present study, it can be concluded that the COP trajectory is different from that of healthy subjects after ACLR. This difference in COP trajectory can result in altered knee loading and increase risk of knee osteoarthritis.
